Ventilação não invasiva para pacientes com DPOC

A ventilação não invasiva (VNI), também conhecida como ventilação com pressão positiva não invasiva (VPPNI), é um tratamento eficaz para a insuficiência ventilatória,1-2incluindo os casos pediátricos.3

Para os pacientes com exacerbação aguda da doença pulmonar obstrutiva crônica (DPOC), o uso da VPPNI está associado a:4-9

  • Redução acentuada da necessidade de intubação endotraqueal
  • Diminuição da taxa de complicação
  • Redução das infecções nosocomiais
  • Redução do tempo de internação hospitalar
  • Redução substancial da mortalidade hospitalar

Embora ofereça benefícios limitados no tratamento a longo prazo de pacientes com DPOC, a VPPNI domiciliar pode ser benéfica em pacientes com DPOC e hipercapnia.10-12

A VPPNI é uma peça importante no controle da patologia para pacientes com doenças neuromusculares e distúrbios da parede torácica avançados. A VPPNI é geralmente iniciada quando há sintomas decorrentes de hipoventilação noturna ou insuficiência cardíaca direita em presença de uma tensão de dióxido de carbono elevada no sangue arterial.13-14 A VPPNI é um tratamento de ótima relação custo-benefício para os pacientes apropriados.15-16
