Vsync and TiControlTM

Closely capturing the dynamic nature of leak, ResMed’s Vsync technology accurately compensations for fast-changing and unstable leak, delivering the right flow at the right time, so that patient–device synchrony is maintained.

  • When Vsync detects a sudden and large change in the leak flow, the usual 10-second window in which it scans for leak, is rapidly reduced towards a 2-second window. This allows for rapid resynchronisation with the patient — usually within two to three breaths, depending on the size of the leak.
  • Once resynchronisation has occurred, the window period is quickly moved back to its default 10-second value.
  • Without Vsync, the outcome would be different. The increased flow from a leak can be misinterpreted as patient inhalation, causing the ventilator to mistrigger from EPAP to IPAP, out of sync with the patient’s actual effort. It can also diminish the ventilator’s ability to cycle the breath.
  • Vsync works closely with the TiControl feature to further maintain synchronisation, even during instances of large leak.
  • TiControl allows you to manage your patient's inspiratory time limit according to their disease state. For challenging respiratory conditions, TiControl allows you to manage spontaneous flow, letting you set your patient’s ideal levels.
  • With its Ti Min and Max settings, you can be rest assured that TiControl safeguards your patient so that they’re breathing at exactly the right pace.
  • Even if your patient’s cycle of breath becomes spontaneous and out of its registered inspiration phase, the device will respond, providing timely intervention.
  • Together, Vsync and TiControl offer extra assurance and control over your patient’s inspiratory time, promoting reliable device-controlled synchrony.

iVAPS: intelligent Volume-Assured Pressure Support

Now you can provide intelligent air through ResMed’s intelligent Volume-Assured Pressure Support (iVAPS). A ResMed technology available on the Lumis 150 VPAP ST-A, S9 VPAP™ ST, S9 VPAP ST-A and Stellar™ 150, it adjusts to a patient’s respiratory rate, targets alveolar ventilation and automatically adjusts pressure support as needed to accommodate each patient’s unique needs, even as their disease progresses.

 

iVAPS with an iBR (intelligent Backup Rate)

Exclusive to ResMed, iVAPS is a volume assurance therapy mode that intelligently monitors whether air is flowing in or out of your lungs, the amount of air that’s flowing through and how much air you need for the next breath.

  • Unlike other volume assurance modes, iVAPS maintains your patient’s unique alveolar ventilation target even when their respiratory rate changes throughout the night (especially during different stages of sleep).
  • Responding to your changing needs, iVAPS provides rapid, yet gentle automatic pressure support that is quick enough to maintain stable alveolar ventilation yet smooth enough to avoid sleep disruption.
  • Using the Learn Targets feature, iVAPS "learns" each individual patient’s spontaneous respiratory rate and uses it as input for the intelligent Backup Rate (iBR).
  • If there are pauses in your patient’s breathing (for instance if they cough or sigh), iBR technology uses the learnt information to determine whether in fact your patient needs help breathing before it delivers a breath for them.
  • This means that your patients will not be over-ventilated, and can have the chance to breathe spontaneously when they can.
  • The ventilator runs the Learn Targets feature and determines each patient’s iBR automatically; you don’t need to manually set it, saving you valuable time and giving you peace of mind that your patient is receiving the personalised ventilation they need.

Triggering technologies for more comfort

Sensitive trigger & cycle technology

To achieve excellent patient–ventilator synchronisation, you can customise the beginning and end of each inspiration with ResMed’s adjustable trigger & cycle technology.

  • Recognising that each patient is different, ResMed provides five levels of trigger and cycle sensitivity to help you tailor and fine-tune triggering and cycling values to different patient conditions.
  • The Trigger sensitivity determines how easy or difficult it is for your patient to initiate a breath. The Cycle sensitivity determines exactly when they start to exhale. The best thing about this breathing solution is that it can be customised to each patient’s unique respiratory needs so that patient–ventilator synchrony is maintained and work of breathing is reduced.
  • The more sensitive Trigger and Cycle settings (High and Very High) provides flexibility for high resistance especially suited to children and those with obstructive lung diseases.
  • Less sensitive settings (Low and Very Low) provide flexibility if your patient is prone to auto-triggering, caused by restrictive lung diseases and cardiogenic artefact.
  • Trigger and Cycle Sensitivities work with a technology called Vsync to accurately measure unintentional leak. Together, these technologies provide optimal synchronisation.

Leak management technology

ResMed's vented therapy solutions offer a range of intelligent leak management technologies.

  • Circuit impedance technologies such as "Learn Circuit" overcome impedance in the circuit and can be found on our Stellar™ series of products.
  • Being able to select the type of mask your patient is using, from the device menu, helps the ventilator factor in mask vent flow (ie, intentional leak).
  • Vsync technology monitors and compensates for unintentional leak and works with TiControl to optimise synchrony.

All this works together to minimise, monitor and compensate for leak and maintain patient–ventilator synchrony, which is the key to effective non-invasive ventilation.

Learn Circuit (Astral and Stellar Series)

Impedance in the circuit varies based on circuit components, including filters, humidifiers, tubing and other accessories. Failing to compensate for circuit impedance can lead to inaccurate monitoring and errors in pressure delivered at the mask, impacting the effective delivery of ventilation.

ResMed's vented therapy solutions are designed to overcome impedance in various ways, depending on the ventilator, either through automatic tests such as Learn Circuit, or manual selection of circuit options via the ventilator menu.

  • At the push of a button and in less than a minute, Learn Circuit automatically runs a test through the ventilator to evaluate any impedance in the circuit.
  • If your patient’s equipment is functioning correctly, you’ll have instant peace of mind.
  • If there is a problem, the device will use this information to compensate for the impedance and ensure that the set pressure is delivered, so your patient’s ventilation is never compromised.

Technologies for personalising therapy

Pathology Defaults (Stellar Series only)

Setting your patient up on a ventilator with ResMed’s Pathology Defaults feature is fast, easy and streamlined.

  • You can choose from a list of pre-set values that are grouped together according to specific disease states and lung mechanics, such as chronic obstructive lung disease, chronic restrictive lung disease, normal lung mechanics, obstructive lung mechanics and obesity hypoventilation.
  • This helps you quickly set up the ventilator with optimised settings for your patient’s disease state, and then fine-tune the pressure settings required for each individual patient.
  • The types of values pre-set by Pathology Defaults are based on commonly used clinical values.
  • How it works: If you select "Obstructive lung mechanics" from Pathology Defaults, for instance, your default settings will be:
     
    • IPAP 13 cm H2O
    • EPAP 5 cm H2O
    • Rise time 150 ms 
    • Fall time 200 ms 
    • Trigger sensitivity Medium 
    • Cycle sensitivity High 
    • Ti Min 0.3 sec 
    • Ti Max 1.0 sec

You can then fine-tune the IPAP, EPAP and rise time as required — and your patient is ready to start therapy.
