Sleep apnea and chin straps: what you need to know

    What is obstructive sleep apnea (OSA)?

    Obstructive sleep apnea is a condition where the muscles around the throat relax to the point of collapse, restricting airflow during sleep.1 Breathing may become shallow or stop for seconds to minutes at a time. Common symptoms of OSA include:17

    • Excessive daytime sleepiness
    • Morning headaches
    • Feeling irritable or moody
    • Difficulty concentrating, struggling to feel present during the day
    • Snoring or gasping for air while you sleep

    The apnea–hypopnea index (AHI) measures how many times your breathing slows (hypopnea) or stops (apnea) each hour while you sleep. Your AHI score helps you and your doctor understand the severity of your sleep apnea. In general, a higher AHI means more frequent breathing interruptions during the night and more severe sleep apnea.

    The severity of obstructive sleep apnea is broken down into three categories: mild, moderate and severe.19 The AHI score helps classify the severity of your sleep apnea.

    What are chin straps, and how do they work?

    Chin straps are fabric bands that go around your head and chin. They’re designed to keep your mouth closed while you sleep.3 Chin straps are usually made from soft materials, such as elastic, and are available in different sizes and styles to help you get a comfortable fit.

    It’s important to understand that chin straps are not designed to address the airway collapse that happens with obstructive sleep apnea (OSA). The impact a chin strap can have on the number of breathing pauses may vary depending on the sleep stage. One study found that a chin strap alone did not have any impact on the number of breathing pauses in rapid eye movement (REM) sleep.4 Overall, research suggests that using a chin strap on its own does not meaningfully improve OSA symptoms.4

    Chin straps and obstructive sleep apnea

    When considering using a chin strap as part of your treatment for obstructive sleep apnea, it’s important to understand what it actually does. While a chin strap may help with snoring,4 snoring doesn’t automatically mean you have sleep apnea. Meanwhile, a reduction in snoring doesn’t mean there’s been a reduction in the severity of your sleep apnea.18

    Very few scientific studies have tested the effectiveness of chin strap use for obstructive sleep apnea (OSA). However, some studies suggest that chin straps may work well when paired with other OSA treatments — specifically continuous positive airway pressure (CPAP) therapy. CPAP therapy uses a mask that fits over your nose and/or mouth, connected to a machine that delivers a steady flow of air. This air is designed to keep your airway open and reduce breathing pauses while you sleep.16 One study suggests that using a chin strap in addition to CPAP therapy may make CPAP more effective.7

    It’s clear that more research is needed in this area, but as of now, CPAP therapy remains the most effective treatment for OSA,15 and chin straps aren’t intended to replace CPAP altogether.4

    Using chin straps in practice

    Chin straps as CPAP accessories

    While there isn’t definitive evidence that chin straps are effective for treating obstructive sleep apnea (OSA) when used alone, they may be used as an accessory to CPAP therapy to help support comfort or fit. They may:

    • Help reduce leaks from a CPAP mask. Some people experience leaks around the mouth when using a nasal CPAP mask. A properly adjusted chin strap can help keep the mask secure and provide a tighter fit for some CPAP users.6
    • Keep air pressure from escaping through the mouth. A chin strap may help some users keep their mouths closed during the night, which may help prevent air from escaping during CPAP use.⁶
    • Help improve the comfort and effectiveness of CPAP therapy for some users. People having trouble adjusting to CPAP may find that adding a chin strap to their setup may help them get a more precise fit and increase the overall success of CPAP therapy. 7

    Chin straps are designed to work with various CPAP mask styles and be an accessible option for those who want to improve comfort and fit when using a CPAP machine.

    Choosing a chin strap for CPAP users

    When it comes to sleep, comfort matters. In fact, people who are happy with the fit and feel of their CPAP mask may be more likely to use their therapy consistently.⁸ Properly fitting a chin strap is important when using it alongside CPAP therapy, as fit may influence comfort and use. Chin straps are available in a variety of materials and designs, and having one properly sized and adjusted by a member of your care team can help ensure a secure fit and reduce slipping throughout the night.7

    If you’re interested in trying a chin strap with your CPAP mask, discuss your concerns and goals with your doctor or CPAP equipment provider before making any changes.

    Limits and concerns

    Chin straps aren’t right for every CPAP user, and it’s important to be aware of the potential limitations and considerations. For example:

    • People with facial hair may find it harder to get a comfortable fit with a chin strap due to it sliding around or pulling at hairs.20
    • For those who tend to move around during sleep, chin straps can also shift and cause skin irritation.8
    • Chin straps may not work as well for those who have nasal congestion.9
    • Chin straps need to be cleaned regularly for proper hygiene and safety.

    Using a chin strap on its own for obstructive sleep apnea (OSA) is not recommended and may pose potential risks, particularly when it delays appropriate treatment.4,9

    What do users say?

    When it comes to what actual users of chin straps think, the reviews are as mixed as the research. While some people find chin straps to be helpful in addition to CPAP therapy, others find them uncomfortable.8 Some users reported less snoring4, improvements in dry mouth symptoms6 and fewer leaks from their CPAP masks.8 The most common concerns from people who tried chin straps were about the fit and comfort.8

    Other options and complementary approaches

    While continuous positive airway pressure (CPAP) remains the most effective and trusted obstructive sleep apnea treatment, there are complementary approaches and tools that may be options for some people.15 For example, oral appliances10 and positional therapy — such as sleeping with your head elevated on a wedge pillow — may help reduce the severity of OSA symptoms.11

    Lifestyle changes, such as weight loss12 and cutting back on alcohol,13 may also help. Chin straps may be used alongside these strategies but they aren’t a replacement for traditional therapies.4,8 It’s important to talk to your doctor if you’re thinking about trying chin straps for OSA.

    When chin straps might help

    Chin straps may help people who:

    • Tend to breathe through their mouths when they sleep.6
    • Experience leaks when using a nasal CPAP mask.6
    • Have dry mouth due to CPAP or breathing through their mouth during sleep.8
    • Need temporary support after a surgery.14
    • Are looking for ways to make CPAP therapy more comfortable.6

    If you’re not sure if a chin strap could help with your symptoms, it’s a good idea to discuss your options with your doctor to determine the right path forward.

    Chin straps may reduce breathing through your mouth or snoring, but they aren’t a standalone treatment for OSA. Always consult your doctor before making any changes to your treatment.
